Notice to Patients of HCAT Boardwalk Diagnostics  ̶  Irving

Effective Aug. 1, 2024, Healthcare Associates of Texas (HCAT) Boardwalk Diagnostics – Irving is ending its imaging services.

Please follow up with your primary care physician (PCP) to ensure uninterrupted care and treatment. It is important to continue your appropriate medical care.

Your medical records will remain at HCAT. You will not be able to pick up your prior films at this imaging location after August 1, 2024. To obtain a copy of your medical records or have your records transferred, please sign a medical records form available at the front desk until Aug. 1, or healthcareassociates.com. Medical information release forms can be mailed to: HCAT Medical Records Department, 6161 N. State Highway, Suite 160, Irving, Texas, 75038. You may also ask the staff at your new physician’s office to fax their release of medical records form to us at 1-972-594-1985, and we will send them a copy of your medical records.
