Healthcare Associates of Texas offers home blood pressure monitoring in conjunction with our partner, Medify Health. This service can help you take control of your cardiovascular health. Most patients on Medicare Advantage and Medicare plans with a secondary plan will pay little to nothing for the blood pressure device and the service.
What is Home Blood Pressure Monitoring?
Home blood pressure monitoring (HBPM) is a preventative healthcare program that helps you keep your blood pressure at normal levels. Patients who are enrolled in a HBPM program take their blood pressure on a regular basis at home using a portable blood pressure cuff. These blood pressure readings are transmitted from the cuff to a health coach who monitors the readings and checks for any changes. Your provider has access to these readings, and can then give you personalized treatment.
How it works
1. A small portable blood pressure cuff is either mailed to you or given to you in our office. | ![]() |
2. At home, you take your blood pressure at least four times per week. Each blood pressure reading is transmitted through cellular service from the device to a health coach. | ![]() |
3. The health coach monitors your blood pressure readings and checks for any changes. If your blood pressure readings are too high or low, the health coach will contact you or reach out to you and your provider. | ![]() |
4. The health coach meets with you monthly over the phone to help you meet your blood pressure goals. | ![]() |
The Benefits
Hypertension can lead to serious health problems that can merit an emergency room visit or a hospital stay. Most patients are unaware of how high their blood pressure has been until they see their doctor during a routine visit. By monitoring your blood pressure regularly, you can work with your provider to prevent heart disease, stroke, and other serious health problems. Having a complete picture of your blood pressure data can also help your provider give you the best care possible. The blood pressure cuff will alert your doctor when your blood pressure is abnormal and they can address the problem right away instead of it going unrecognized and untreated for months.
